19.1 PURPOSE

ISM Code 12.0, 13.4, 13.8, / ISO 9k- 9.2/ ISO 14k-9.2

The purpose of this provision is to ensure that the company shall establish and maintain a program of documented procedures for conducting SQEMS audits at planned intervals to determine and verify that all activities conform to the planned arrangements of the SQEMS; and that the management system is being properly implemented, maintained, and is effective.

19.2 SCOPE

This provision shall apply to all internal audits of activities regulated within this SQEMS that are carried out in the Company office and on fleet vessels.

19.3 RESPONSIBILITY

The HSSEQ dept shall be overall responsible for planning and executing the audit programme with support from the DPA/MR.

19.4 SAFETY, SECURITY, QUALITY, ENVIRONMENTAL & NAVIGATIONAL AUDITS

a)SQEMS audits shall be a systematic and independent investigation & appraisal to include:

  1. Effective implementation of the SQEMS to guarantee Safety, Quality, Navigational & Environmental standard achievements in accordance with the intended goals;
  2. The SQEMS adaptability and adherence to changing requirements as imposed by legislation and/or by other regulatory bodies.
  3. Identification of scheduled audits criteria, scopes, frequency, method, and objectives;
  4. Recording and reporting of results;
  5. Performance of corrective or preventive action in accordance with documented procedures;
  6. Follow-up activities shall include the verification of the actions taken and the reporting of verification results;
  7. Responsibilities and requirements for planning, conducting, reporting, and maintaining records;

b) SQEMS audits shall be carried out per a present plan by qualified and independent personnel at a frequency of a minimum of one audit of all activities ashore and on board per year. In exceptional circumstances and with approval of the DP/MR, this interval of twelve months may exceed by no more than an additional three months. Selection of auditors and conduct of audit shall ensure objectivity and impartiality of the audit process. Auditors will not audit their own work.

c) The results of the internal audits shall be brought to the attention of all personnel having responsibility for the activities audited.

d) Management personnel having responsibility for the areas audited shall take timely corrective action (e.g. “without undue delay”) on deficiencies found.

19.5 VERIFICATION

a) The purpose of the Company's verification activities is to ensure that legal and regulatory requirements as well as the Company's policies, documented system procedures, and instructions are adhered to.

b) The verification activities include Safety, Quality, and Environmental aspects in shipboard working processes and maintenance, as well as onshore activities of significance.

c) All management and supervisory personnel, both shipboard and onshore, have the responsibility and authority to verify the sufficiency of Safety & Quality regarding work, activities, systems, equipment, and services in their area of responsibility.

d) A manager or supervisor who deems a subordinate competent for verification of specific work, system, equipment, and services can authorize the subordinate for such verification.

e) The DP/MR of each group has the responsibility and authority to verify and report any substandard condition related to Safety, Quality, and Environmental aspects.
